We want to hear your views and experiences, as someone who uses health and care services and/or cares for someone who does!
In Leeds, and across the country, the NHS and Social Care want to prevent people spending unnecessary time in hospital or care homes. Doing this will involve strengthening services like GP practices and care in the community, and connecting people to other public services and voluntary sector services.
Leeds Community Healthcare Trust are interested in understanding your experiences of accessing health and care services both for yourself and with the person you care for. They also want ideas for how this experience could be improved.
It would be fantastic if you could spend 5 minutes answering this short survey. The findings of the survey will be used to inform changes to community health and care services in Leeds. A report will be written based on the survey findings. Individual responses will be anonymised.
